It is essential for patients to complete the history in advance to facilitate a worthwhile and efficient meeting with the medical provider. |

| For those who do not have access to the internet at home or work, internet access can be obtained free of charge at all local public libraries. We also provide workstations for history entry at our office, but use of these workstations requires early arrival for your visit. |
